1. WHAT IS ROE (RADIOLOGY ORDER ENTRY)?

Radiology Order Entry (ROE) is a web-based application that allows referring physicians to electronically order and schedule outpatient MRI, CT, Ultrasound, Mammography, Plain Film Radiography, Nuclear Medicine, PET, PET/CT, Bone Densitometry, Spine and Joint Injections, and Nuclear Cardiology Exams. ROE is accessible from any workstation inside the Partners' firewall.


2. HOW TO ACCESS ROE

From a web browser, type http://mghroe into the address bar of a browser and press enter to go to the ROE Home Page. (www or .com is not required - this is an internal web page)





For first-time users, a brief one- time registration is required. You will be redirected to a registration screen, please follow the instructions below. If you are a clinician and do not see your name in the Registrars database drop down list after you choose “clinician” please contact Bill Barron 617-643-2198 or Michelle Dean 617-643-2494.





If you a Registered Nurse or in a non-clinical role, ROE will ask you to fill in your e-mail address and name.





3. HOW TO BOOK AN EXAM

From the ROE home page, select the ordering physician from the drop down list.





Enter a valid patient MRN and click “Next>>”





The “Select Exam” screen will be displayed (1).
A list of any pending (ordered and / or scheduled) radiology exams for the patient will be displayed at the bottom of the screen (2).





From the horizontal menu bar, click on the type of exam you would like to order (1).  The menu will drop down to display the specific exam types.  Click the exam to order (2).





The exam order page will be displayed.  Complete the order, providing relevant information about the patient by using the check boxes and text boxes. One indication from signs and symptoms and/or known diagnoses must be checked off in order to proceed.

Click “Save Exam” to complete the order.





4. WHAT IS ROE DECISION SUPPORT?

After saving the exam information, the Decision Support screen will appear for all high cost imaging exams such as CT, MR, and Nuclear Cardiology. You will only be directed to the decision support screen when ordering high cost imaging. Review the information and then click “Proceed” to order the exam.  Click “Cancel” to discard the order and select a different exam.





5. HOW DOES DECISION SUPPORT WORK?

The clinical information provided at the time of ordering is combined with patient age to produce a “utility score” for the examination requested.

The scores have been derived from published scores, especially the American College of Radiology Appropriateness criteria. They have been modified and are continually being reviewed by MGH clinical and radiology experts. Possible scores range from 1 (very low utility) to 9 (very high).

A high score indicates that the information provided strongly supports the use of the imaging test requested.

If you are not the ordering physician and your exam requested receives a low utility score (1-3), the exam will then require a physician’s approval.

A low score (1-3) indicates that the information provided does not support the use of the imaging test requested. The user will be asked to review the indications and be prompted to provide a reason to proceed with the order.


6. HOW TO PROCEED

This will require the ordering physician to log into ROE and review this exam. A  “review” button will appear to the left of the exam in the pending exam queue for this patient. S/he will be required to review the clinical indications and either update any necessary information (calculating a new decision support score) or complete a free text field explaining the reason for proceeding with a “red score” exam.

The decision support tool is intended to provide information to medical practices in utilizing Radiology exams in their most efficient manner.





7. HOW TO SCHEDULE AN EXAM

To schedule an exam, click the “Schedule” tab.





The “Schedule” page will appear.  Click the “Schedule” button next to the exam.

The “first available” appointment will display.  If one of the first available times works for the patient, select the date & time and click “Schedule” (1). If a first available time is not acceptable, Search for times by selecting a location from the pull down menu and then click “Search”. The appointment location is the only required field (2).






The “ROE Calendar” will appear. For those days with appointment times available, the number of available appointments will be displayed on the calendar.






Click on a day that has available appointments. The exact times will appear.  Select the desired time (1) and click the schedule button (2).





The exam is now scheduled. You will now be returned to the “Schedule” page. You may print (1) the exam instructions for the patient by clicking the “print” button (next to the exam you just scheduled).  You may also re-schedule by choosing the re-schedule button (2) for a different date & time, or cancel the exam.
